Start here for your first visit.

Book online, then finish your intake forms before you arrive so check-in is faster. Returning patients: see the form rules below if it has been a while or you are seeing a different practitioner.

Your checklist

New patients: please complete booking and all online forms before your visit whenever possible.

    Book your appointment

    Choose a time online through our secure scheduling system, or call the clinic. Tell us if you are a new patient so we can allow enough time.

    What to expect online

    • The booking scheduler is on our booking page. Pick a service, practitioner, and time.
    • Then enter your name and email to finish the booking. Creating a password / account is optional (you can also sign in if you already have one).
    • Use a real email and phone number when you book so we can reach you. If you get stuck, call the clinic and we’ll book you.
    • After you book, come back here for intake forms (step 2). Forms are separate from booking — finish them before you arrive.

    Complete new-patient forms

    Health history and consent stay in our clinic forms system — not the public contact form. Please complete all of the forms below before your visit so we spend less time on paperwork at the front desk.

    Returning patients: you will need forms again after three months (minor assessment), after one year (initial exam again), or whenever you book with a different practitioner. If you are unsure which apply, call us.

    A couple of items (for example a hand-drawn symptom diagram) may still need to be finished in clinic until our online versions are ready. Complete everything you can online first.

    What happens at the visit

    • Consultation: discuss your concerns and goals. The legacy site described a complimentary first conversation; confirm current policy when you book.
    • History & exam: your practitioner assesses movement and relevant tissues. Imaging only when it would change the plan.
    • Plan: clear findings and options. No guaranteed outcome; you decide next steps together.

    • Comfortable clothing you can move in
    • Relevant imaging, reports or referral information
    • Insurance details if you plan to submit for reimbursement
    • A list of medications or health concerns

What forms do I need to fill out?
New patients should complete intake forms before arriving. These include health history, consent, and contact information. Forms are available online through the new patient checklist.
How should I prepare for my first chiropractic visit?
Wear comfortable clothing that allows movement. Arrive a few minutes early, bring any relevant imaging or reports, and be ready to discuss your health history and current concerns.
